Vital Records

Fulton County Board of Health - Vital Records

The Fulton County Vital Records Office acts as the local agent for providing certified birth and death certificates for Fulton County residents. We can also print and issue certificates for births and deaths that occurred in any Georgia county. At least 95% of these records can be printed on demand, but in some cases additional wait times might be necessary and can take up to 72 hours before it is available for printing.

Services and Fees

Georgia law and the Department of Public Health regulations require that all requests for vital records include the signature, picture ID of the requestor, and payment of the proper fee.

The following options are available for requesting a vital record:

Order Online

VitalChek accepts payment by Visa (debit or credit), MasterCard (debit or credit), American Express and Discover. Exact fee information is available online.

Request By Mail

Mail a completed copy of the Birth Certificate Request Form or the Death Certificate Request Form, required supporting documents as described in the form, and payment to:

Mail-In Payment Options: Certified check and money order. All should be made out to “Fulton County Board of Health Vital Records”. Please do not send cash in the mail.

Mail-In Fees: $25 non-refundable search fee

  • Includes a certified copy of the record, if found
  • Additional copies of the same record ordered at the same time are $5 each (full-size available only)
  • Georgia law requires pre-payment before a record can be provided

We only provide records of vital events that occurred in Georgia. For events that took place outside of Georgia, please contact the vital records office in that state or territory for more information. The National Center for Health Statistics maintains contact information for vital records offices throughout the United States.

For Local Funeral Homes wanting to pick up, please fill out the form below by 3:00 PM daily. Please be sure to indicate if you would like to pick up at the Government Center Location OR College Park. All payments are due upon pickup via Cash, Debit/Credit (No Amex), or Personal Check (No Counter Checks or Post-dated Checks).
