COVID-19 Vaccinations

Fulton County encourages all eligible residents to get a COVID-19 vaccine and booster shot(s). Vaccines are safe and effective at preventing serious illness and death from COVID-19.

 * NOTE: If you are getting your second vaccine dose, or your booster vaccine, please bring your Vaccination Card to your appointment/visit.

We also offer free, at-home services for those Fulton County residents who are more vulnerable to COVID-19 infection and death. For more information, please visit our At-home Vaccinations and Testing for Vulnerable Populations.

Vaccine Times and Locations

COVID-19 vaccines are available for residents ages 6 months and up at the following locations. In addition, booster vaccines are available at all health centers for ages 6 months and up. Appointments are encouraged, but not required.

Vaccine Consent Forms

Minors between 12-15 years of age need to be accompanied by a parent or guardian, or person designated by the parent/guardian, and the parent/guardian’s consent is required.

Youth ages 12-17 do not have to have a parent present but must have a consent form signed by a parent.

Insurance

We accept the following insurance carriers:  CIGNA, AETNA, BLUE CROSS BLUE SHIELD, ANTHEM, AMBETTER, HUMANA, MEDICAID, and UNITED HEALTHCARE.

We will also see those who are uninsured.
